There is a great and ever increasing socioeconomic burden on Europe due to the prevalence of people with multiple chronic diseases. CLOSE serves to innovate homecare for severely ill, insulin treated patients with type 2 diabetes mellitus (T2DM) by offering an integrated product and service package (AP+). Blood glucose levels will be controlled by an artificial pancreas (AP) that will be installed at the patients' home, operated through trained caregivers, and remote controlled (telehealth). CLOSE will provide a French test scenario, which paves the roads between prescribers, patients and payers and demonstrates effective, safe and cost-efficient operation of AP+. CLOSE plans to then tailor AP+ to other homecare systems and reimbursement policies for deployment across Europe. The ambition of CLOSE is to improve homecare for patients, alleviate strain on supporting relatives, prevent further health complications, and reduce treatment costs.

The common goal is shared by private, public and community actors to create a healthier life. The Health Movement will prevent chronic diseases from emerging, and increase quality of life for individuals that are already sick. Based on the individual’s preferences and situation, the Health Movement creates an ecosystem of community actors to help the individual lead a healthy lifestyle, comply with treatment and get personalised care. Health index provides means for the region to follow up health on a population level and is the base for reimbursement to service providers.

However, there is a gap between sport practice in the society and its use in the medical domain. Sport4Health aims to fill this gap by connecting sports associations, healthcare professionals and patients. It will provide sport prescriptions, safety check and feedback through a digital environment. The project will focus on the deleterious association between sleep apnea and obesity.

| WarmUApp Project Type: Innovation by Design Project Leader: Sanofi-Aventis (France) Partners: IESE Business School (Spain), Oxford University (UK), CGI France SAS (France), BePatient (France), Hyphenstat (France) Duration: 12 months (01/01/2016-31/12/2016) Non Communicable Diseases (NCD) are stressing health systems and affecting companies directly through health cost and losses of productivity. Recently, European companies have been trying to tackle this issue through health management programs. Nevertheless, health outcomes are hardly quantified. To ensure the long‐lasting investment of these costly wellness programs, it is vital for companies to implement efficient interventions and evaluate scientifically their effects on 1) Employee lifestyle behavior changes, 2) Health cost and absenteeism. The objectives of WARM‐APP (Assess, Monitor & Adjust your Wellness Program) are to provide a solution for companies implementing wellness programs at the workplace. This solution empowers managers to assess the real impact of their investments on their employees’ health behaviour changes and permits to adjust their interventions to match their ROI expectations.
